Discharge Planning Project Scoring Rubric

Introduction: Discharge planning begins at the point of entry into the hospital. Third party payers as well as clients themselves demand quality care that includes well-planned, smooth, and timely discharge. It is our responsibility as professional nurses to begin the discharge plan, to possess a fundamental knowledge of available resources, and to continually change the plan of care based on the status of the client.

Objective: Learners will develop and clearly communicate a comprehensive discharge plan of care that demonstrates critical thinking and accurately links community and medical surgical theory to practice.

 Guidelines:

1.       Each student will work alone to complete this project.

2.       The discharge planning project will be presented in the form of a PowerPoint presentation. Either recorded narrations or live virtual presentations (TBD) will be required to support details of each slide.

3.       Citations should be provided for information that is not the student’s own – i.e. pathophysiology, stats, etc. Personal communications should also be cited. Please follow APA for citations.

4.       A reference list for citations should be included at the end of the PowerPoint presentation.

—        One PowerPoint slide should address each element that is bulleted with a blank box – i.e.(20 Blank boxes)

5.       Submit the PowerPoint presentation on the assigned date.

6.       Your presentation will be scored using the Discharge Project Presentation Scoring Rubric and the points earned will be entered into the course gradebook for CET week 15.

7.       Students are encouraged to read and follow rubric in developing their projects and to note point deductions for critical errors noted at the end of the rubric.

Consideration should include but is not limited to the following areas that the student will investigate for effective discharge planning (rubric below includes all areas):

  • Education in potential food and drug interaction, and diet modification regarding the underlying cause of the admission to the hospital
  • Rehabilitation techniques and support systems needed after discharge
  • Community agencies that the client and family can utilize
