To Prepare:

  • Review the Resources and identify a clinical issue of interest that can form the basis of a clinical inquiry.
  • Review the materials offering guidance on using databases, performing keyword searches, and developing PICO(T) questions provided in the Resources.
  • Based on the clinical issue of interest and using keywords related to the clinical issue of interest, search at least two different databases in the Walden Library to identify at least four relevant peer-reviewed articles related to your clinical issue of interest. You should not be using systematic reviews for this assignment, select original research articles.
  • Review the Resources for guidance and develop a PICO(T) question of interest to you for further study. It is suggested that an Intervention-type PICOT question be developed as these seem to work best for this course Advanced Clinical Inquiry and PICO(T) Questions.

By Day 3 of Week 4

Post a brief description of your clinical issue of interest. This clinical issue will remain the same for the entire course and will be the basis for the development of your PICOT question. Describe your search results in terms of the number of articles returned on original research and how this changed as you added search terms using your Boolean operators. Finally, explain strategies you might make to increase the rigor and effectiveness of a database search on your PICO(T) question. Be specific and provide examples.

Searching Databases

Nursing is the biggest clinical segment in the United States, accounting for nearly 40% of operational costs. Nurses are obvious targets for cutbacks and decreased workloads. The restructuring of healthcare and elderly populations has expanded the hospitalized patient population, and the shortage of nurses has had a great effect on patient experience and outcomes. As such, the PICOT question for this study is as follows: in hospitalized patients (P), does improving nurse staffing levels (I), compared to not addressing nursing shortage (C), lead to enhanced patient experience (O) and outcomes in 3 months (T) Advanced Clinical Inquiry and PICO(T) Questions.

The CINAHL Plus, Walden Library, and MEDLINE were the main databases I utilized to look for information. Firstly, the search terms I utilized in my study were nursing shortage, nurse staffing, and nurse-patient ratio. I chose exact terms since these are the essential ideas. The search yielded 8,408 items in the CINAHL Plus database, which is a large number of papers to deal with in the real world. I limited the results of the search to publications from 2016-2021. By utilizing the Boolean operators, I was able to narrow my search. According to Grewal, Kataria & Dhawan (2016), one may filter their search by employing Boolean terms like AND, OR, and NOT if they conduct research and obtain too many results. There were 1531 peer-reviewed articles as a result of this.

Other techniques, like utilizing database filters, are used to improve the accuracy and efficacy of searching for information in databases. Database filters should be used to sift the information collected (The University of Texas, 2020). I utilized advanced searching in the MEDLINE database, like “overcoming nursing shortage,” and set it up to generate search results under the “health information” category. The number of results that came up as a result of this was 345, which is more workable. On the same database, I was able to use the right-hand search widgets to show several widgets depending on important ideas. I filtered the articles using the search widget to obtain the most pertinent articles and current ones Advanced Clinical Inquiry and PICO(T) Questions.
