The proper delivery of the healthcare system is mainly based over three concerns. These are the need for care, its quality and its cost. However, these concerns are being catered by the healthcare systems throughout the country. The quality of the healthcare system is based over the delivery of care, quality and cost. The transfer of tasks from doctors to nurses has a reducing or neutral costs. By cons, advanced practice nurses can lead to increased costs for the following reasons such as; a longer consultation, a larger number of users of references to other physicians, increasing the rate of repeated consultations and sometimes prescribing a greater number of tests. When the new advanced practices are additional tasks not performed by doctors, the incidence appears to be increasing the cost of care. Obstacles or factors favorable to the development of advanced practice nurses Interests of professional bodies In most countries, the body’s resistance medical professional is recognized as one of the main obstacles to the development of advanced practice nurses. To mitigate this resistance, some countries have adopted strategies of collaboration between the various professional associations to define the respective practices of various professional bodies (Hamric et al, 2000). Organization and financing of care In some countries, health care services are organized around one doctor, while in other contexts, care is provided by professional groups associated with several professions. The mode of organization and method of payment of care affect the expansion of advanced practice nurses. In many countries, legislative constraints limit the role of nurses e.g. prescribe medication or to play certain roles in primary care. The development of advanced practice nurses work requires adaptation of legislative and regulatory framework. The deployment of advanced skills nurses must also be based on the adaptation of training programs. In some countries, new programs must be created. The OECD countries are at various stages of the implementation of advanced practice nurses, some countries have already formally adopted new roles, and other countries, by cons, still exploring the possibilities. Thus, countries that are studying the introduction of new practices have the leisure to learn from others’ experiences in adapting to their environment. It should however be borne in mind that, even in countries that have formally adopted advanced practice nurses, the number of nurses involved in these changes is relatively small compared to the total number of nurses.
