4-2 Final Project Milestone Two: Health Promotion and Disease Prevention and Management Strategies.
Submit Section II of the final project case study, in which you will address the organization\’s health promotion and disease prevention and management strategies. Specifically, you will critique the organization\’s healthy living programs, disease prevention and management programs, and non-programmatic services. You will assess the cultural competence of these services using the population health approach. Be sure to consider issues such as how well the organization\’s health promotion and management strategies address a wide range of cultural beliefs and whether the organization\’s programs and non-programmatic services help to reduce inequalities among members of the population. Evaluate the cultural competence of the healthcare organization presented in the Integrated Safety-Net Health Care System case study using a population health approach. Please also read the supplemental Interpreting Services Program document to ensure that you have all of the information about the Montefiore Medical Center you will need to address the critical elements.
Specifically, the following critical elements must be addressed:
- Patient engagement activities and communication strategies
- a) Critique the organization’s translation and interpretation services that may be available to non-English-speaking patients. In other words, does
the organization provide such services? How many different languages are supported? How might the organization communicate with a speaker

Efforts to Promote Healthy Living
Montefiore Medical Center is focused on providing its patients with programs for a healthier lifestyle. For instance, personnel performs healthy cooking demos in the facility waiting room to help solve the nutrition issues experienced by this vulnerable demographic, especially those associated with obesity. Additionally, they appoint health instructors to manage the reading scheme for children. The facility efficiently hires and retains a staff of Vietnamese, Cambodian, and Hispanic descent to help address community issues related to basic cultural competence. The organization partnered with the area parks team to give recreation center memberships to known diabetes patients for free. Montefiore leadership launched a weekly market for farmers on Montefiore ground to provide the community with fruits and vegetables and has worked with community organizations to enhance green markets across the Bronx. In order to offer counseling and education to encourage self-management in patients, five approved diabetes educators move around Montefiore Medical Group zones (Chase, 2010)
Appropriateness of the Organization’s Disease Prevention and Management Programs and Services
There was a framework established by Dr. Kvetan referred to as the “ICU without walls” scheme with respect to disease prevention and management. This program gives clients the chance to gain input from system-wide acute care medical specialists. According to the article, the interdisciplinary panel of doctors, patients, and other medical professionals gather at 7:00 in the mornings to examine every patient, accompanied by the critical care consultation unit in the ICU and different hospital departments. There are approximately more than 100 of these clients daily. The multidisciplinary team evaluates a care plan that is then inputted with an acuity score into a system ICU registry as well as the details of the guiding physician in charge during that process. In the past few years, yearly acute care visits have declined by 20% from a record of approximately 5,000, and average mortality rates at Montefiore declined to 1.76% in 2008 from 3.5% in 1997 (Chase, 2010)Assignment 1 Week 2 Health Promotion
Non-programmatic Services
The Montefiore Medical Center has thought creatively about means of delivering their services to clients in relation to non-programmatic services. The organization has established Patient-Center Medical Homes using funds from The Commonwealth. Medical homes are meant to improve access to healthcare for disadvantaged individuals and minimize preventable trips to the emergency room (Chase, 2010). This offers more patients to have access to services without appointments and yet meeting critical needs.
Gaps or Deficiencies
It appears that Montefiore Medical Center goes beyond its boundaries to serve its community. They pursue means of helping as many individuals as possible, including homeless individuals, people with or without health insurance, and many more. Nevertheless, the only services that seem to be missing are services to do with childcare. This is an immense requirement, both for staff and for clients. When patients require care, if they are guardians, there may be a significant limitation in the patient who needs to get the care they require, recognizing that they do not have assistance with their children.
