Dementia represents a broad category of brain diseases, where an individual fails to retrieve either short term or long term memory or both. This condition affects daily functioning in the life of an individual. The symptoms that are commonly associated with dementia are impaired cognitive, and communication skills that lead to episodes of forgetfulness. Dementia or the act of chronic forgetfulness can occur due to various reasons, and the most common causes are the neurodegenerative changes associated with aging. Dementia often occurs as a concurrence to Alzheimer’s disease, which represents the pathophysiological basis associated with the condition (Cunningham, 2006).
The hippocampus and its associated structures of the limbic system, play a significant role in the development of emotion, and the formation of memory. Whenever any neuronal pathways to these structures are damaged or become redundant, memory formation and retrieval of old memories become compromised. In Alzheimer’s, there is a significant functional loss of cholinergic neurons, in the Nucleus Basalis of Meynert, due to the development of neurofibrillary entangles around these neurons. As a result, acetylcholine is not formed and cannot carry information to the hippocampus for the development of new memory. Discussion & Conclusion
Performance Improvement Plan is intended to either improve the services of an existing operation for better productivity and safety, or it should create such services in addition to some existing services that will create a value addition in the mind of clients. Our PIP will revolve around the concept that Dementia patients are prone to fatal accidents due to falls, and this is potentiated by the gap in communication and person-centric care by care providers.
To address such issue in our hospital we insist on a pilot study and history records of safety in these patients which will introspect the reasons for such incidences. The core quality and safety management team will comprise of the Chief operating officer (Administration) along with RN and Nurse Managers. The roles of Nurse Managers are vital because they will be the chief carriers and implementers of the vision and mission of the caregiving the process to the nursing staff. Our PIP will focus on the development of skills and upgraded knowledge regarding treatment of patients suffering from Dementia through evidenced based practice and expert recommended guidelines.Effective Performance Improvement Assignment
The PIP will be implemented and measured through the Quality management steps and will be robust in acting upon the feedback of the caregiving process.
