Gynecologic Health History Assignment Discussion Paper
The first thing a nurse does in the process of providing care to a patient is to obtain that person’s health history. Involvement in this process necessitates the amassing of vital data, which is then used in formulating the most appropriate therapeutic strategies. The ability to build a patient’s medical history is, as a result, an essential competence that ought to be possessed by all healthcare professionals. An important part of getting to know a patient’s medical history is posing pertinent questions that are tailored to their unique situation (Slade & Sergent, 2018). As part of this assignment, I will be providing a script for an interview to be conducted involving a volunteer who will be acting as a patient Gynecologic Health History Assignment Discussion Paper

Reflection
The drafting of the interview script is very necessary in order to guarantee that all pertinent questions are not left out. The process of creating the script required a high level of technical expertise as well as reasoning skills. According to Ingram (2017), having a clear grasp of how to obtain a health history frees the brains of medical personnel, allowing them to make use of their crucial expertise and skills. This is of the utmost importance when asking sensitive and challenging questions. Additionally, a realistic image of a clinical situation to be addressed is created.
I must admit that despite the importance of being able to complete a sexual history taking and ask all the pertinent questions, I ended up feeling inadequately prepared to do so. The procedure was made more difficult for me by the fact that I had to deal with a number of concerns, including differences in culture, the dread of being offensive, and shyness. It was difficult for me to ask questions that may potentially be embarrassing to the patient, and I was very aware of the need to avoid doing so at all costs. As such, it was challenging to ask the patient questions about their sexual health, like whether or not they have sexually transmitted diseases and what sort of sexual activities they are engaging in (Leyva‐Moral et al., 2020). This led to the elimination of some items because I was feeling uneasy Gynecologic Health History Assignment Discussion Paper
Conclusion
Taking a patient’s health history is a crucial ability that should be mastered by all healthcare professionals. The collection of patient-reported information is the goal of building a health history. This will enable the patient and the medical personnel to work together to devise a strategy that will improve the patient’s overall health, treat any current health issues, and reduce the risk of developing any chronic medical issues

For this Assignment, you will develop a script to be used to interview a volunteer serving in the role of patient. To prepare: Review the Ewing (2004) questionnaire found in this week’s Learning Resources and consider the difficult questions you might have to ask when you take a patient’s complete health history. Review the screening tools found in the Learning Resources and consider how you might use an app or tool to assist in screening. Review the media programs related to a vaginal exam, pap test, and breast exam. Review the health history guide presented in Chapter 6 of the Schuiling & Likis (2017) text and consider how you would create your own script for building a complete health history. (Note: You will also find the Health History Form in Chapter 6) Gynecologic Health History Assignment Discussion Paper

Provide all the components of a complete gynecologic health history. Include considerations for special populations such as LGBTQ+ individuals. What health maintenance guidelines should be included for initial and follow up might be needed for follow-up assessments? (i.e., bone density test, Gardasil vaccine, shingles, etc.)? What questions would you consider in your patient’s complete health history? For example What is your patient’s living situation? Do they have stairs? Do they live by themselves? Do they have a working refrigerator? Develop your own script for building a complete health history and as you create your script, consider the difficult questions you want to include in your script. There is no sample template to provide to you. (Utilize chapter 6 of your Schuiling textbook to provide guidance). You are the one to develop the script. Think of it as you are writing a movie and you need to write the script for the movie. What lines would you provide for the actor to utilize when sitting down with a patient to perform a COMPLETE Medical History which also entails those DIFFICULT GYN questions. You do not need to provide the answers to the questions however, if you find that beneficial, you may do so. Assignment: (1- to 2-page reflection) In addition to your script for building a health history for this assignment, include a separate section called “Reflection” that includes the following: A brief summary of your experiences in developing and implementing your script during your health history. Explanations of what you might find difficult when asking these questions. What you found insightful and what would you say or do differently.
