Intervention Presentation and Capstone Reflection Essay Paper
Part One
Mr. Turner and his family were the subjects of my Capstone project, in which I recognized hypertension as a health issue and engaged with them. I chose my elder uncle, who is 58 years old since he belongs to the high-risk category and would be able to identify the core reasons causes and devise appropriate management measures for the situation. Achieving a reduction in the number of deaths and morbidities within this population, as well as improvements in the quality of care, efficacy, and patient safety is important in lowering the overall cost of care for the person, the healthcare organization, and the government.

Health education on the DASH diet, home blood pressure monitoring, and healthy behaviors like quitting smoking and regular exercise, were some of the interventions I adopted for hypertension. The client was fully engaged in decision-making and his own treatment as a result of these interventions. In addition, feedback from the patient and his family were used to design focused care approaches. Integrating telemedicine facilitated efficient nurse-patient communication and cooperation, thereby improving the overall safety, quality, and effectiveness of treatment delivered (Alley et al., 2021). By utilizing phone reminders and alerts, as well as increased patient and family engagement, telemedicine was able to solve medication compliance problems (Li et al., 2020) Intervention Presentation and Capstone Reflection Essay Paper.
Part Two
Contribution of the Intervention to Patient or Family Satisfaction and Quality of Life
The client fully engaged in the assessment procedure and stated his desire to change his lifestyle in order to manage his blood pressure levels. He also had good responses from his family as a result of the interventions’ affordability, universal application, and the capacity to comprehend the health state of their loved one. The patient complied with all of the recommendations I made for maintaining low blood pressure. He said that with a blood pressure monitor, the likelihood of missing medication doses is decreased, and he can live a healthier lifestyle. He engages in regular physical activity, which includes jogging and daily walks. He has also embraced the DASH diet, which is essential for maintaining a heart-healthy living. Having a healthy weight, engaging in physical activity, minimizing drinking, and stress management are all recommended in conjunction with the DASH diet (Kang & Park, 2018). In addition to achieving optimal health outcomes and setting individual health objectives, such interventions enhance the client’s overall life quality.
Client satisfaction and experience were improved as a result of good communication and cooperation between the client, family, and medical professionals. The client was able to video contact his providers via the use of telemedicine, which resulted in better blood pressure management and a reduction in admission risk. For successful hypertension treatment to be achieved, the client must have had continuous care, self-monitor, and have had easy accessibility to care throughout this timeframe. Medical errors were decreased because of better care coordination, which resulted in higher quality care. As a result, the client and his family were content with the treatment they received and were enthusiastic about continuing the self-management activities Intervention Presentation and Capstone Reflection Essay Paper.
Use of evidence and peer-reviewed literature to plan and implement the capstone project
Throughout my capstone project, I examined current evidence-based literature on hypertension, which helped me to understand better the health interventions that were put in place. Upon researching the disease, I learned that factors including high sodium intake, insufficient intake of fruits and vegetables and a sedentary lifestyle, as well as excessive drinking might all contribute to the ailment’s progression. I also learned that high blood pressure might contribute to the onset of chronic illnesses such as heart attack, stroke, dementia, metabolic syndrome, aneurysm, and a range of other conditions. Furthermore, the literature search allowed me to get a better understanding of a few of the risk variables, like age, obesity, ethnicity, family medical history, and others that may be adding to the disease’s development. This capstone project has provided me with the opportunity to collect sufficient evidence-based techniques that may aid in the prevention of hypertension. As such, I got to learn that a person’s health may be jeopardized if they consume an excessive amount of sodium. Individuals must be educated on such risk factors in order to prevent them.
Healthcare Technology
Health technology was used as part of my capstone project in order to enhance patient-provider interaction and to promote self-management of high blood pressure. The client’s ability to use technology to track his health condition and implement healthy living habits was critical to the success of the project. Due to the fact that the client could effortlessly monitor his blood pressure with an application but had trouble utilizing telemedicine technology, I focused on videoconferencing sessions. As part of my role as a healthcare practitioner, I worked with the client to ensure that he could receive care and medical services at home. Telemedicine enabled me to address emerging problems with the client, provide clarification on the Dash diet, and provide health education, all via a single platform. The patient and family made excellent use of technology to get accessibility to cost-effective and high-quality remote healthcare. Just so medical professionals could stay up-to-date with patient management strategies, they used electronic medical records to guarantee efficient data gathering, retention, and information exchange Intervention Presentation and Capstone Reflection Essay Paper.
Health Policy
Policies in the field of health guide the choices, strategies, and activities made to accomplish care and patient objectives (Mileski et al., 2017). During the course of this project, I examined healthcare policies that have an effect on nursing practice as well as the usage of technology in the healthcare setting. The Affordable Care Act (ACA) and the Health Insurance Portability and Accountability Act (HIPAA) were two policies that appeared to be significant. Regulations under the Health Insurance Portability and Accountability Act (HIPAA) regulate the use of technologies in patient care in order to guarantee that patient data is private, secure and to increase patient safety (Cohen & Mello, 2018). In order to guarantee cost-effectiveness, the project was developed as per the Affordable Care Act. As part of my education, I informed the family about the rules and procedures in place to ensure their protection and the confidentiality of their information, thus increasing the usage of technology and the desire of the patients to partake. For this reason, I consulted with the nursing staff and other specialists in the field to guarantee that safe, and quality treatment is delivered. As a registered nurse, my goal is to guarantee that health policies are designed with patient requirements and preferences in mind and implemented.
Capstone Project Outcomes and Initial Predictions.
The end results of my capstone project were in line with my expectations. Because of the increased mortality and morbidity rates, I concentrated on hypertension. I also learned that older people are at a higher risk of developing hypertension. To enhance health outcomes within this population, I sought to develop and execute efficient strategies to resolve low compliance with treatment and unhealthy behaviors. The self-monitoring of blood pressure and other Dash diet self-management measures have been improved with the use of telemedicine. Apart from that, the client has begun to participate in health promotion activities and gets the total backing of his family. The fact that the intervention is cost-effective has had a significant influence on the other individuals who engage in the focus groups in which the client attends. As a member of the group, I have seen a pleasant atmosphere as well as patient contentment in helping other people. Physical activity and drug-free rehabilitation were part of the patient’s treatment plan, which included following the DASH diet recommendations. This assisted in addressing the risk factors for high blood pressure as well as the effects associated with high blood pressure Intervention Presentation and Capstone Reflection Essay Paper.
Improvements in care coordination, as well as good communication and teamwork within the healthcare system, also occurred, which is consistent with my expectations. As a result of my interactions with health professionals, I have gained a new perspective on the need for an improved healthcare sector in which one’s specialty and credentials are less important than the energy and vigor with which treatment is administered to people. Optimal health outcomes and improved patient care are achieved via inter-professional cooperation (Shimbo et al., 2021). Achievement in enhancing the health of my patient as a result of my interventions will lead to their implementation into the healthcare system. Drawing on Covid-19 approaches, technology is used in many areas of care to minimize social contact and guarantee that susceptible people are properly catered for. Mobile applications may be used to manage chronic illnesses like high blood pressure, thus reducing costs and helping the government meet its other health-related goals.

Personal and Professional Growth
Taking on the capstone project has made a significant contribution to both my personal and professional development. I have learned that it is critical to maintain a professional demeanor in my working life. First and foremost, I learned that respect for other people is essential in the profession. I gained a greater appreciation for my coworkers, patients, and other individuals. When one shows respect to another, such respect is returned to the one who showed it in the first place. I also learned that it is impossible to succeed in the healthcare sector unless you operate as a team. The ability to collaborate with other medical practitioners is critical for improving patient outcomes. Consulting doctors, including dietitians in the safe care of clients, involving pharmacists in interventions, and several other things were taught to me throughout my time in the project.
In terms of my personal growth, I have learned that I have to progress in my profession and study a lot about the safety and the many ways individuals may prevent illness. Learning and research, to a great extent, are required to accomplish this goal. As a result, it is my ambition to go further in my professional life. Aside from that, I have become increasingly spiritual. We often feel compassion for the sick until we pray for them. Certain diseases are difficult to cure, and in addition to providing treatment, providers may also provide spiritual advice to patients. I’ve also learned that patients need someone who can defend their rights. As a result, I have become a strong advocate for several patients in our facility Intervention Presentation and Capstone Reflection Essay Paper.
