Reporting of notifiable diseases is a crucial source of health information. In my state, a disease is reported if it has the potential to cause severe morbidity or death and if it has the potential to spread, and can be controlled with proper approaches. Doctors, nurses, and other liable health workers are involved in reporting any infectious disease that comes to their attention. Additionally, my state requires reporting by householder, hospital, or individuals having information about a case of an infectious disease. In hospitals, a particular health professional is in charge of putting forward the reports. These required reports can be individual case reports or collective reports (reports of a group of cases).
