Population Health Improvement Plan

Introduction

In the recent past, the United States is facing increasing numbers of chronic illnesses among the citizens. Such chronic diseases include cancer, stroke, heart diseases, and HIV/AIDS. Currently, high numbers of deaths are caused by chronic illnesses and other diseases. More so, the US health department incurs more than $3.3 trillion annually to cater health care costs among residents (Bobinski et al., 2018). Even though the number of patients is escalating, some risk factors are catalyzed by risk behaviors such as excessive drinking and poor nutrition methods. These cases have made government and non-governmental organizations join efforts to prevent and control chronic diseases through health management measures. This paper underscores HIV/AIDS as a chronic health issue based on demographic, environmental, and epidemiological evaluation in employing an evidence-based population health improvement plan in the community.Population Health Improvement Plan

Besides, the main concern on the HIV/AIDS pandemic revolves around the methods on how to manage and control the disease from spreading to uninfected individuals. Like, the spread of HIV/AIDS remains a significant concern for improving the community. In this issue, African American infections are the primary concern in the community. The application of a probable initiative towards the community will fit in regard to demographics, epidemiology, and environmental factors. Likewise, after establishing a community plan, it is important to set up-bottom means of communication that will easily uphold health goals for African American families in an ethical way.Population Health Improvement Plan

Community Demographic Evaluation

According to statistics, it is approximately one million citizens in the United States lives with HIV. In a group of seven people there are chances one person is unknowingly infected with the virus. For instance, in 2016, the figures indicated that around 30,000 people had contracted the virus. The main affected groups include the bisexuals and gays since they are vulnerable of getting the disease. Later on, the number of infected black and African American increased after every year. In addition, transgender women is a factor to consider on vulnerability since it put the group at risk of getting infected.

Epidemiological

In the United States, the pandemic has different stains that spread unevenly across the states. Still, some of the pandemic strain have characteristic based on the initial geographical areas that affect the community depending with the frequency of the risk factors. In addition, some biological factors influence the infection by increasing or decreasing the patient’s susceptibility to the virus. Therefore, this alters normal progress of the disease and sometimes hastening the spread of the virus and death. The epidemiology of HIV indicates that the number of infected people without their knowledge is increasing. For instance, the research shows 1 out of 5 people in US are infected unknowing. This creates a total of 21% of the total number of infected individuals.

Environmental data

HIV/AIDS pandemic it’s a devastating pandemic that not only affects a section in the globe, but currently infect more than 33 million peoples live with the virus (Iacob, Iacob & Jugulete, 2017). The disease continues to spread its impacts in every sector thus affecting people’s livelihood such as communities, families, and individual level. In the US, the disease spread in the context of environmental factors that has taken place as a result of societal dynamics. For instance, human industry and climate changes have taken place due to human adjustments that are caused by poverty, conflict, and inequality thus affecting the disease self-sustaining environment. However, this can be termed as a major reason to spread the virus in the community since these factors are not yet properly explored.

In addition, the pandemic has revealed to have more environmental attachment that influence the spread. Such factors include presence of natural resources, effects of climate changes, workforce, gender, livelihood, and food presence. These factors make it difficult to elevate the chronic illness from the community since there is intertwined connection between HIV/AIDS and the environment. The phenomena between environment and the disease has remain undiscovered since there is no concrete framework that conceptualize the relationship.Population Health Improvement Plan

Environmental factors affecting the health of community residents

Environmental factors affect the choices that the community takes, thus influencing health ideas in different families. Still, the community’s ability to select a healthy option is regulated by the surrounding where they work, live, and play. There are various environmental factors affecting community health. They include financial security, community safety, housing, education, and employment. Some of the elements are directly related to personal health, while others are indirectly related. This mean the community can control some of the factors while others are naturally beyond control.

Inadequate resources in the surrounding make the community dependent on handling health matters. For instance, poverty is a factor linked to community health, thereby affecting food insecurity, natural resources, and HIV/AIDS. At the community level, most people cannot cater their health bills as well as having proper feeding due to poor economic and social status. Lack of social determinants greatly affects the community health of individuals living in chronic conditions. For example, an environment with poor accessibility of social factors such as education is underdeveloped, making residents prone to illnesses.

Ethical health improvement plan

In the community, the surrounding has various barriers that affect a health issue. Majority of the patient infected with HIV/AIDS hassle to get care, face stigmatization, high medical cost, transportation cost, and patient relationship (Abdulrahman et al., 2019). The environment social barriers play a critical role in maintaining community health in check. This community health initiative, there is the need to direct some attention to environmental, social factors that act as barriers to the productive living of people with HIV/AIDS. Develop an ethical health improvement plan that effectively addresses the population health concern that you identified in your evaluation of the relevant data

Such an intervention is crucial at the community level since patients will continue to rely on community intervention established primarily to support people with chronic illnesses. This is significant to the majority of the local people since they are familiar with local issues. Various duties that previously seemed to require clinical skills are subdivided into volunteers with the aid of professionals. This plan at the community will help the members lower unnecessary expenditures incurred while seeking medication.

Additionally, community health workers plan adds values to the residents while offering HIV programs such as accompanying patients to clinics, making referrals, linking them to hospitals, and offering psychological support. Evidence-based evidence shows that the community plan will have mutual support from both sides since both create an enabling working strategy that is needed. Such an environment includes supportive supervision, equipment, and moral support for the elderly in the community.Population Health Improvement Plan

Propose criteria for evaluating population health improvement plan outcomes

Evaluating HIV/AIDS community-based health improvement program is crucial in ensuring the project runs within stipulated objectives and goals. The establishment of this community project is to lower the number patients who continue with the medical diagnosis program. The proposed plan will lower the incidence rate by a certain percentage since people will get educated on coping with the disease. Such themes will be taught in the community plan including the preventive measures, community-based home visits, street outreach intervention, peer education, and family-based HIV preventive measures. Directing effort to the community-based program will lower the number of illnesses associated with the pandemic. This can be employed as an evaluation and testable mean that can be applied in the community to gauge the proposal performance success.

Communicating strategy with colleagues and members of the community

Community project requires optimal communication to take place while solving various issues in different departments. For instance, in the HIV/AIDS community-based intervention, colleagues offering services need to listen to one another to encourage patients to visit the premises with an interactive mood. Also, colleague communication decreases misunderstanding that might erupt while working together. A good relationship creates a good working environment with a healthy relationship that adds value while solving internal matters. Besides, the community members can raise their problems and their experience with medical experience. The intervention includes communication in regular meetings, thus including both groups. As well, the community members will be able to interact with workers and will share their internal pressing issues.
