I undertook my practicum at a nursing and rehabilitation facility that cares for older patients. The patients in this facility were aged individuals in need of exceptional care. I practice under the oversight of my coach who has always provided me with the greatest support. My rational reasoning and analytical skills significantly improved because being a nurse practitioner, when working individually, I have to consider certain stuff. Week 1

New Practice Approaches

During my first week at the rehabilitation facility, I got a significant chance to acquire knowledge and skills associated with the quality of treatment to aged people with various illnesses. I learned that a nurse nurse practitioner needs significant experience and knowledge in the treatment of vulnerable populations. I observed that there is a need for specific interventions to support patients in the process of recovery, which includes the usual functioning of the body, dietary intake, and agility (Kim, 2016). Additionally, patients require cognitive, emotional, and spiritual support. I came to the realization that patient care is affected by basic things such as hygienic practices. Nursing entails patient care. It includes a pragmatic mechanism that assists with the recovery of patients. through the practicum experience, I learned several things that I assumed I understood, but I did not actually know. For example, I believed that caring for patients was a core feature of the nursing field but I was able to learn through the practicum the career in nursing has various components, including prevention of infections and diseases, palliative care, and emergency services (Schmidt et al., 2017). Week 2

Interprofessional Collaboration

I discovered how critical inter-professional collaborative teamwork in healthcare is during this week’s practice. I found that interprofessional cooperation among nurses, physicians, pharmacists, nutrition experts, and other healthcare professionals helped them to connect at a deeper level and exchange information regarding their patients as well as ways of dealing with them. Interprofessional teamwork allowed the interdisciplinary team members to engage in cooperation to better the situation of patients. They exchanged thoughts about ways to address the different needs of patients and ways of overcoming the health issues faced by these individuals (Murdaugh et al., 2018). furthermore, they encouraged me to come up with ways of solving falls in this group of patients. I made sure that I was involved in the operations of the interdisciplinary team throughout the practicum, where we tried to find the most suitable means to handle the conditions of different patients. For elderly patients, I focused on discovering evidence-based approaches and prevention methods for falls.

Week 3

Healthcare Delivery and Clinical Systems

During this week, I acquired expertise in the facility so I can fully appreciate the essence of healthcare. The primary purpose of nursing is to minimize and help people get back to normal life. The facility centered on relieving psychological, spiritual, emotional, and physical suffering (Goswami 2017). The purpose of the practicum experience was to offer essential care and rehabilitation to elderly patients. It also addressed the patients’ desires and needs. The facility utilized a care provision program similar to those used in home care since the facility was used for elderly patients.

Week 4

Ethical Considerations in Healthcare

When I was doing my practice in the facility this week, I discovered that nurses had multiple ethical issues, which ranges from hospice to the end of life care. The ethical considerations that I encountered in my practice include inadequate communication, inadequate treatment, decision-making, confidentiality for the patients and the utilization of opioids, issues related to lifelong-term drug discontinuation, and managing symptoms (Ellis 2019). even so, healthcare providers can effectively handle these concerns intelligently with a clear ethical code.

Week 5

Population Health Concerns

Through thorough observation of patients in the facility, I found that most of the patients seeking care included the aged and the critically ill. I also observed that many of the patients had more problems, including falling, weak coordination, and walking disabilities. Elderly individuals with a rich background were well off than lower and middle life classes in regards to the state of health (Fonta et al., 2020). Week 6

Health Policies

During this week, I learned that doctors who collaborate with doctor assistants and nurses in the setting are working towards determining suitable treatments for end-stage patients. By collaborating, they make sure that patients access the medicines they require to treat their condition and alleviate pain (Murdaugh et al., 2018). The framework also offers information on pain relief and management of different conditions prior to visiting healthcare facilities.

Week 7

Role of Technology in Improving Healthcare Outcomes

I found that technology assists in healthcare delivery during this week’s practice in the care facility. EHRs also brought about a consolidation of patient data, making accessibility to information convenient for care providers (Ellis, 2019). moreover, technology has strengthened the research nursing and the application of evidence-based practice in care facilities, thus improving healthcare delivery.

Week 8

Leadership and Economic Models

Both leadership economic designs were important elements that were systematically explored during my practicum this week. There have since been developments in leadership skills. During my preparation, I significantly developed my decision-making and leadership skills. I have enhanced my communication skills, as I needed them to interact with the interdisciplinary team members (Ellis 2019). Enhanced skills in communication also made me comprehend directives, thus enhancing my practice. Finally, because we functioned together, I have strengthened my social cohesion and teamwork.

Week 9

Health Disparities

During my practicum experience, health disparities became a dominant focus. I found that the racial disparity between black and white patients was high. Many patients were white compared to blacks. This was attributed to black people being treated poorly. The wealthy and poor are another significant health disparity (Harvey et al., 2015). The majority of patients in the facility came from well-off families. A strategy to accommodate all patients in health facilities must be established within the health system.
